What we're building
CarePath is a chronic care management service for people living with long-term conditions (diabetes, hypertension, COPD, and similar). A patient shares their health goals, lifestyle, current medications, and life situation — and the service returns personalized, practical alternatives: care routines, habit adjustments, and support options that fit their real life. It is supportive guidance and care coordination help, explicitly not a substitute for medical advice.

A landing page and a "Get my care plan" intake flow that turns a patient's conditions, goals, lifestyle, medications, and life situation into a personalized care-plan summary with tailored alternatives (deepened per-condition and goal content for diabetes, hypertension, COPD, heart, asthma).

Pay-per-plan (live): a one-time $19 for the full personalized plan, collected through a Stripe checkout link with a Buy button on the plan page. No real charge has landed yet (Stripe shows $0, no transactions) — the first confirmed sale is still pending.
